The New Patient Appointment (30 - 60 minutes) consists of a complete Consultation, Exam, Orthopedic and Neurologic testing. Diagnostics may be recommended: X-Ray or MRI.
Next Day Report of Findings (30 - 60 minutes)
It only takes about a day to process the consultation, exam and diagnostic images (X-Ray, MRI, …) from the new patient appointment. Your findings and recommendations will be presented to you along with a report folder for your records. The Doctor may want to run additional testing or will begin treatments that day.
Please bring with you:
- The completed Patient Application Form . If your injury or pain is associated with an auto accident, please alternatively complete the Personal Injury / Auto Case Check List .
- Any images (X-Ray, MRI…), as long as they are not older than 2 years.
- For Scoliosis Patients: Additionally, complete the online SRS-22r Form.
- For auto accidents/personal injury cases: Additionally, complete the oswestry and QOL form
